- Birth Certificate
- Shot Records
- Physical (dated within 1 year)
- Parent/Guardian Photo ID
- Proof of Residence (utility bill, rent/lease agreement, must be more than a regular piece of mail)
- *If your child is available to attend registration, testing can be completed at that time!
PreK
- Must provide proof of income
- Must be 4 by August 15th
- Must live in Chester County
- Pick up applications at the Board of Education between 8:00-4:00
